Medication Reconciliation

Medication reconciliation is a proven method of making sure a patient is receiving appropriate medications as they transition between different points of care. The attached links provide useful tools for the medication reconciliation process. The most difficult part of medication reconciliation is determining a patient’s current medication list. To help with this the toolkit provides different options for a patient to use in recording their medications.
